Indications/Uses
Adults w/ community acquired pneumonia, nosocomial pneumonia, or complicated skin & soft tissue infections that is known or suspected caused by susceptible gm +ve bacteria.
Dosage/Direction for Use
Max treatment duration: 28 days. Community acquired pneumonia & nosocomial pneumonia 600 mg bid for 10-14 consecutive days. Complicated skin & soft tissue infections 600 mg bid.
Administration
May be taken with or without food.
Contraindications
Patients w/ uncontrolled HTN, phaeochromocytoma, carcinoid, thyrotoxicosis, bipolar depression, schizoaffective disorder, acute confusional states. Concomitant use w/ SSRIs, TCAs, serotonin 5-HT1 receptor agonists (triptans), directly & indirectly acting sympathomimetic agents (including adrenergic bronchodilators, pseudoephedrine & phenylpropanolamine), vasopressives (eg, epinephrine, norepinephrine), dopaminergics (eg, dopamine, dobutamine), pethidine or buspirone; MAO-A or B inhibitors (eg, phenelzine, isocarboxazid, selegiline, moclobemide) or taking any medicinal products w/in 2 wk. Lactation.
Special Precautions
Myelosuppression. Close monitoring of blood counts in patients who have preexisting anaemia, granulocytopenia or thrombocytopenia; receiving concomitant medications that may decrease Hb levels, depress blood counts or adversely affect platelet count or function; have severe renal insufficiency; receive >10-14 days of therapy. Wkly monitoring of complete blood counts (including Hb levels, platelets & total & differentiated leucocyte counts) regardless of baseline blood count is recommended. Should only be used in patients w/ complicated skin & soft tissue infections w/ known or possible co-infection w/ gm -ve organisms if there are no alternative treatment options available. Discontinue treatment if antibiotic-associated diarrhoea/colitis is suspected or confirmed. Lactic acidosis. Inhibits mitochondrial protein synthesis. Closely observe for signs & symptoms of serotonin syndrome eg, cognitive dysfunction, hyperpyrexia, hyperreflexia & incoordination. Peripheral or optic neuropathy; increased risk of neuropathies w/ concomitant use of antimycobacterial medications for TB. History of seizures or risk factors for seizures. Overgrowth of non-susceptible organisms. Patients w/ diabetic food lesions, decubitus or ischaemic lesions, severe burns or gangrene. Concomitant use w/ MAOIs is not recommended. Consuming large amounts of tyramine-rich foods. May impair ability to drive or operate machinery. Severe hepatic insufficiency. Pregnancy. Childn <18 yr. Elderly.
Adverse Reactions
Candidiasis, oral/vag candidiasis, fungal infections; anaemia; insomnia; headache, taste perversion (metallic taste), dizziness; HTN; diarrhoea, nausea, vomiting, localized or general abdominal pain, constipation, dyspepsia; abnormal liver function, increased AST, ALT or alkaline phosphatase; pruritus, rash; increased BUN; fever, localized pain; increased LDH, creatine kinase, lipase, amylase or non-fasting glucose; decreased total protein, albumin, Na or Ca; increased/decreased K or bicarbonate; increased neutrophils or eosinophils; decreased Hb, haematocrit or RBC count; increased/decreased platelet or WBC count.
Drug Interactions
MAOIs. Mean increase in systolic BP w/ pseudoephedrine or phenylpropanolamine HCl. Serotonin syndrome-like effects w/ dextromethorphan. Serotonin syndrome w/ serotonergic agents including antidepressants eg, SSRIs. Avoid ingesting excessive amounts of food & beverages w/ a high tyramine content (eg, mature cheese, yeast extr, undistilled alcohol beverages & fermented soya bean products eg, soy sauce).
